Records & Confidentiality
All communications between student and staff are strictly confidential. Information will not be released from the Student Health Center without the patient's prior approval except in an emergency or by court order.
Medical Records
The SHC maintains patient records from 1995 to the present. Current students' medical records remain active for two years after graduation or otherwise leaving University of Richmond. The information is then scanned onto a disk for electronic storage. Only pertinent medical information, such as immunization records, encounter notes and test results, is retained.
The SHC will respond to all routine requests for medical information within five working days of receiving a signed request. If the SHC deems a request for the release to relate to a medical emergency or urgent situation, we will respond as soon as possible.
The SHC will not accept requests for medical records by e-mail. For legal reasons, a patient must mail or fax a written request stating what is needed and where it is to be mailed or faxed. Required on the request are your name while at UR, signature, the date, your date of birth, University of Richmond student ID number, and a phone number where we can contact you with any questions.
Cost for this service varies depending upon the number of pages printed and postage, but generally does not exceed about $3. Record requests for non-patient entities are calculated using different parameters.
For the convenience of our students requesting medical information or giving us consent to release medical information, the form for Authorization for Release of Medical Information may be downloaded, completed, dated, signed, and faxed to the proper destination(s). If you are a former student requesting records, please be sure to fill in the blank asking for your date of graduation or separation from the University. Please notify us if your name has changed since you were a student.
