Health Record Requirement
In compliance with state law, the University of Richmond requires all new, entering full-time students to provide documentation relative to their health and immunizations.
As a matter of policy, the Student Health Center requires non-UR students residing in University housing for more than one month to comply with state law regarding documentation of required immunizations. Please submit the Health Record requirements listed below (TB Risk Assessment form and Immunization Record) to the Student Health Center prior to your arrival on campus.
Health Record requirements for entrance includes completion of the following:
- TB Risk Assessment Form (completed by the student on-line through Bannerweb: Student Services:Health Record Requirement)
- Immunization Record (No Physical Exam required)
Deadline for submission of the TB Risk Assessment and Immunization Record:
- Fall semester, all students—July 1
- Spring semester, international students—December 1, transfer or re-admitted students—January 5.
Students not in compliance with the above Health Record requirements by the deadline will have class registration and residence hall access blocked.
