Summer 2009 Information
The Student Health Center is closed to patient care for the summer, but we are working administratively. If you are unable to find the information you need on our web site, our administrative staff checks and responds to emails and phone messages weekly throughout the summer. Health History Record
In compliance with state law, the University of Richmond requires all entering full-time students to provide documentation relative to their health and immunizations.
ALL students must have forms on file in the Student Health Center by July 15th. This provides time for us to review the forms and return them to you if any areas remain incomplete or require additional information. Undergrads who fail to return the completed form will have class registration and residence hall access blocked. Noncompliant law students will have their network accounts locked on Tuesday of the first week of classes. Noncompliant grad students will have class registration blocked. Mail the completed form IN ITS ENTIRETY to the Health Center. Faxes will not be accepted.

Required information is to be completed by a health care provider. Required:- 2 MMRs (measles, mumps, rubella-2 doses of each)
- Tetanus booster - within past 10 years
- Polio series
- Hepatitis B (3 doses or signed waiver)
- Meningococcal meningitis vaccine (or signed waiver)
- Tuberculosis screening - Required for ALL students (see forms for details)

Prescription Refills
If you are requesting a refill on a prescription written by one of our physicians, please call and leave a message or email us with your name, medication and dosage required, name and phone number of your pharmacy, and a phone number where we may reach you if necessary.
Medical Records
If you need a copy of your immunization history or medical records, you may download our Medical Release form. Please mail or fax your request, including year of graduation, name when you were UR student, SSN or UR ID number, date of birth, and an address or fax # where you would like us to send those records. This request must be signed and dated. Please include a phone number where we may call if we have questions.

Should our students who stay in the Richmond area need allergy shots, vaccines, or other medical services during the summer, the following information is provided as a suggestion for your convenience. Please call the facility for current prices.
