Pre-Entrance Health Requirements

Deadline: Fall Semester – July 15  Spring Semester – January 15

The University of Richmond adheres to the immunization and tuberculosis screening requirements of the Commonwealth of Virginia. All full-time students new to the University are required to complete pre-entrance health forms prior to campus arrival. Students enrolled in the part-time MBA or part-time SPCS programs are not subject to this requirement.

Requirements: MMR, Tdap or Td, Meningitis, Polio, Hepatitis B, Tuberculosis Screening

Step 1:
Obtain a copy of your immunization record with the preceding vaccinations. Click here for paper form if needed. All documentation must be in English.

Step 2:
Manually input immunization dates from your record to the electronic form via your Student Health Center Portal, complete the COVID ROI, and answer the Tuberculosis screening questions.

Step 3:
Include physician signed copy of immunization record via the 'upload' button on your form, or via the 'Document Upload' tab of your Student Health Center Portal.

Step 4:
Complete Medical History form via your Student Health Center Portal.

Use your University Net ID and password to complete and submit all electronic forms on your Student Health Center Portal.

Communication occurs by secure message, not email, via your Student Health Center Portal. Check the 'Messages' tab often for communication from the Student Health Center staff.

AVOID THE HOLD

Any student with incomplete records after three weeks of classes will have a hold applied to their Student Account to block registration for next semester classes. The hold will remain in place until all record requirements are met. University of Richmond Policy states: The required documentation must be submitted by July 15 for fall admission and January 15 for spring admission for review by the Student Health Center staff. Any student with incomplete records will be contacted via a secure message in their Patient Portal with specific instructions for satisfactorily completing the process.
